1. Death Certificates on Demand

a.    Applicant/informant to the death could be relative to the deceased or anybody present at death

b.    Applicant/informant to complete Form NR10 (Death Report) obtained from the District Registration Offices in all the 28 District Councils

c.    Particulars to be verified by Village Headman and Senior member of the village or Religious  leader, where death occurred

d.    Applicant/informant to submit Form NR10 form to District Registration office

e.    District Registration office to check for completeness and Issue Acknowledgment.

f.     District Registration office to send Form NR10 to NRB for data entry
