• Births Occcuring in Health Facilities Open or Close

    1. Where parents are married

     a.    Applicant/informant to the birth could be mother, father, relative or anybody present at birth

     b.    Applicant/informant to complete Form NR8 (Birth Report) obtained from the Health Facility

     c.    In case of twins or more, separate forms for each child shall be filled

     d.    Name of the child and details of the father of the child shall be provided

     e.    Absence of one of the parents shall not hinder registration of birth

     f.     Any birth occurring in health facilities shall be registered once it has occurred or upon discharge of the mother

     

    2. Where parents are not married

     a.    Applicant/informant to complete Form NR8 (Birth Report)

     b.    Name of the child shall be provided

     c.    In case of twins or more, separate forms for each child shall be filled

     d.    Details of the father of the child shall only be included upon mutual consent of both mother and father of the child (both shall sign on the Form) or upon production of a court order confirming paternity of the child

     e.    Where the biological father has denied paternity, mother shall register child under their maiden [surname] names

    1. Where parents are married

     a.    Applicant/informant to the birth could be mother, father, relative or anybody present at birth

     b.    Applicant/informant to complete Form NR8 (Birth Report) obtained from the Village Headman

     c.    In case of twins or more, separate forms for each child shall be filled

     d.    Name of the child and details of the father of the child shall be provided

     e.    Absence of one of the parents shall not hinder registration of birth

     f.     Village Headman and a Senior member of the village or a cleric of a religious institution shall certify the birth by signing on the Form

     g.    Births shall be registered in the village/area in which it has occurred

    1. Where parents are married

     a.    Applicant/informant to complete Form NR8 (Birth Report) obtained from the District Registration Office

     b.    Hospital report of birth shall accompany the application

     c.    Birth Report form shall be verified by the Village Headman and a Senior member of the village or a cleric of a registered religious institution in which the parents or one of  them is a member

     d.    District Registrar or an authorized person shall verify the correctness of the particulars in Form NR8 as submitted by the applicant/informant

     e.    District Registrar/authorized person shall request attachments such as copy of passport, driving licence, air ticket, doctor’s appointment letter, college admission letter, or a  combination of these documents to accompany the application

     f.     In case of twins or more, separate forms for each child shall be filled

     g.    Name of the child and details of the father of the child shall be provided

     h.    Absence of one of the parents shall not hinder registration of birth

     i.      Birth shall be registered in the district/area in which it occurred

     

    2. Where parents are not married

     a.    Requirements as in [3 i (a-f)] shall apply

     b.    Name of the child shall be provided

     c.    Details of the father of the child shall only be included upon mutual consent of both mother and father of the child (both shall sign on the Form) or upon production of a  court order confirming paternity of the child

     d.    Where the biological father has denied paternity, mother shall register child under their maiden [surname] names

     e.    Birth shall be registered in the district/area in which it occurred

     b.    Village Headman and a Senior member of the village or a cleric of a religious institution shall certify the birth by signing on the Form

     c.    Birth Report form shall be verified by the Village Headman and a Senior member of the village or a cleric of a registered religious institution in which the parents or one of  them is a member

     d.    District Registrar or an authorized person shall verify the correctness of the particulars in Form NR8 as submitted by the applicant/informant
